International providers are under no obligation to file claims on behalf of our members. You may need to pay for the services at the time you receive them and then submit a claim along with an itemized receipt to us for reimbursement. If you’re overseas and have a U.S. VPN, you can download the 2026 BCBS FEP Vision brochure at bcbsfepvision.com/brochure or visit our website at www.bcbsfepvision.com to obtain a claim form or to submit a claim in the member portal. If you don’t have a U.S. VPN, you can email us at fepmemberhelp@davisvision.com with any benefit questions or to obtain a claim form. To file a claim for covered vision care services received outside the United States and Puerto Rico, send completed claim forms and itemized receipts to:
